अच्छी diabetes care केवल glucose number तक सीमित नहीं है। Kidney, retina, nerves, feet, heart और blood vessels में changes शुरुआत में बिना symptoms के हो सकते हैं।
Kidney
Clinical plan में urine albumin और blood creatinine/eGFR शामिल हो सकते हैं। Urine में protein दिखने या creatinine बढ़ने तक symptoms का इंतजार नहीं करना चाहिए। BP और कुछ medicines भी kidney protection में महत्वपूर्ण हो सकती हैं।
आंख
Retinal examination केवल चश्मे की power check नहीं है। अचानक vision loss, नए floaters या curtain जैसा दिखना urgent eye assessment मांगता है।
पैर और nerves
पैर की skin, nails, sensation, footwear और circulation की समीक्षा useful है। रोज cuts, blister, redness या swelling देखें। Sensation कम हो तो गर्म पानी या heating pad से burn हो सकता है।
Heart risk
BP, cholesterol, smoking, activity, weight और family history को glucose के साथ manage करना चाहिए। Screening interval सबके लिए समान नहीं; diagnosis की अवधि और reports के अनुसार doctor तय करते हैं।
अपनी अगली diabetes visit के लिए checklist
HbA1c या glucose reports के साथ urine, creatinine, lipid profile, eye report और पुरानी prescriptions उपलब्ध हों तो साथ लाएँ। घर की sugar readings में fasting, meal के बाद की reading, low-sugar symptoms और medicine का समय note करें। पैरों में सुन्नपन, burning, night pain, wound या footwear से रगड़ की बात जरूर बताएं।
Tests का उद्देश्य डर पैदा करना नहीं, बल्कि complications को उस समय पहचानना है जब बचाव या treatment अधिक प्रभावी हो सकता है। Normal report का मतलब यह नहीं कि future screening बंद कर दी जाए। दूसरी ओर हर visit पर सारे tests दोहराना भी जरूरी नहीं। Type of diabetes, diagnosis की अवधि, pregnancy, kidney result और पिछली eye/foot findings के अनुसार schedule अलग होगा।
क्या न करें?
पैर के corn या nail को blade से न काटें और wound पर तेज chemical न लगाएँ। Kidney report खराब हो तो पानी, protein या medicine में बड़ा बदलाव internet advice से न करें। आंख की vision अचानक कम हो, पैर लाल-गर्म हो, तेजी से swelling बढ़े, blackening दिखे या wound से pus आए तो routine follow-up की प्रतीक्षा न करें।
