Care scope
Every visit is structured around a careful clinical review, a clear plan, and appropriate follow-up.
Multiple chronic illnesses requiring one coordinated plan
Six or more daily medicines or prescriptions from several doctors
Dizziness, falls, low sodium, low BP or suspected medicine side effects
Memory concerns, confusion or changes in daily function
Weakness, weight loss, poor appetite and nutritional risk
Balance, walking and frailty concerns
Diabetes, BP and kidney-risk review in older adults
Influenza, pneumococcal, shingles, hepatitis B and other adult vaccines
Review the person’s goals, daily function, cognition, nutrition and caregiver context—not only individual diagnoses.
Reconcile prescriptions carefully and reduce unnecessary medicines only when it can be done safely with the relevant treating teams.
Create a practical prevention plan covering falls, bone health, vaccines, screening, advance planning and specialist coordination.
Healthy-ageing guide
Geriatric care prioritizes what matters most to the person while reducing avoidable treatment burden.
Older adults often experience several conditions at once, and the safest plan may differ from treating each disease separately.
Taking many medicines is sometimes necessary, but duplicate or interacting prescriptions can cause harm.
New confusion, progressive memory difficulty, recurrent falls or unexplained weakness deserves assessment rather than being dismissed as normal ageing.
Unintentional weight loss and reduced appetite can increase infection, falls and recovery risk.
Vaccination can reduce preventable infection and hospitalisation, but timing depends on age, previous doses and medical risk.
Visit flow
Patients are guided through assessment, explanation, treatment decisions, and follow-up with calm, practical communication.
Choose the clinic that is most practical for continuing care and follow-up.

These answers are general information and cannot replace an individual medical assessment.
Look for assessment of function, memory, falls, nutrition, medicines and caregiver needs in addition to individual diagnoses. Dr. Pratyush Kumar is a former Senior Resident in Geriatrics at PMCH and has DNB Family Medicine training.
Sometimes, but deprescribing requires diagnosis review, current symptoms, kidney and liver function, treatment goals and coordination with other prescribers. Blood-pressure, steroid, seizure, psychiatric and several other medicines should never be stopped abruptly.
Occasional forgetfulness can occur, but progressive memory loss affecting medicines, money, navigation, safety or daily activities deserves assessment. Sudden confusion is different and may be a medical emergency.
Common discussions include influenza, pneumococcal and shingles vaccines, with hepatitis B, COVID-19 and other vaccines based on age, previous doses, medical conditions and current Indian recommendations and availability.
Many benefit from influenza, pneumococcal and other risk-based vaccines because chronic illness can increase complication risk. The exact schedule depends on age, disease, medicines and vaccination history.
Seek urgent care after a fall with head injury, inability to stand, severe pain, new weakness on one side, speech change, loss of consciousness, sudden confusion, major bleeding, chest pain or breathlessness.
